Transcripts

To request that a transcript be sent to a College or post-secondary institution, these 2 online forms must be completed.

 First, a parent or guardian must complete the release included in this link:

**THIS STEP ONLY NEEDS TO BE COMPLETED ONE TIME**

Second, the student must complete the transcript release form found here: 

Transcript Request Form (Links to an external site.).  Please retain your confirmation of submission after submitting the form.
