    All forms listed below must be completed by the first day of practice, August 4th, 2025.

     

    Register on the Google Form (link below) FamilyID is OUT!! No longer!! (Complete By Monday August 4th,2025), the Team Rules (turn in the first day of practice), PIAA Physical Form must be completed and brought to the Athletics Office or the Athletic Trainer by 8-4-25 and your Report Cards must be presented to Coach Foley on the first day of practice in order to practice. Failure to bring these forms in will be one less day for you to be evaluated.

     

    Physical Forms 2025-26-This form must be completed and turned in the Athletic Office or the Athletic Trainer by August 4th, 2025.  The physical must be dated AFTER May 1st, 2025. The physical will be honored for the entire 2025-26 school year.

     

    You CANNOT practice without a completed physical,non-negotiable!!
