    Physicals must be turned in BY AUGUST 4TH

    SECTION 6 MUST BE SIGNED BY A DOCTOR AND DATED AFTER MAY 1 2025

    CONCUSSION EDUCATION/BASELINE TRAINING:

    • 9th grade, 11th grade, transfer students. Are the only athletes that need to take the BASELINE 

     

    • EVERYONE MUST COMPLETE THE EDUCATION FORM.
