• All incoming 7th Graders are required to have a TDAP AND Menactra Vaccines PRIOR to entering 7th grade. Please get both immunizations this summer, if your student has not already received them. 
• All 7th grade students are required to have a dental examination and a completed dental form returned to the school health office.
• All 6th grade students are required to have a physical examination and a completed physical form in the school health office.
• All medications, prescribed and Over-the-counter, require a Doctor's order at the beginning of each school year. If you desire your student to be administered any medication while at school, please have your doctor fill out the medication order form before returning to school in the Fall.
