Transportation Forms

Please use the forms below to submit the appropriate request or information for Transportation related concerns.

1. Non-Public School Transportation Request Form
Act 372 of 1972 requires that students attending non-public or private schools that require transportation to submit this form on an annual basis.

2. Transportation Change Request
If you are requesting a change to your child's transportation, please use this form to submit your requests. Please note that submission of a form does not mean that your request will be approved. Each request is viewed on a case by case basis and a decision is made.

3. Split Custody Transportation Request
Used to request transportation to the second parent's address due to a court ordered split custody arrangement. Both Parent's must reside in the same area serviced by the school. A copy of the decree will need to be submitted.

4. McKinney-Vento Request
For use by School Districts and other public entities who need to submit for McKinney-Vento Transportation.
